Who needs a Financial declaration?
The Financial Declaration is usually used in the Superior Court of California for the matters of child support, maintenance, fees, costs or debts. The declaration is completed by the respondent and must be filed with the court clerk.
What is the purpose of the Financial declaration?
The Declaration provides detailed information about the declaring’s total monthly income, household expenses, and debt expenses. It is used at the court hearing to determine an adequate amount of payment due to the other party (petitioner). The form also contains some personal information about the declaring.
What documents must accompany the Financial declaration?
If the court hearing concerns' child support, the declaring should attach the completed Washington State Child Support Worksheets along with the Confidential Information Form. Besides this, they should separately send the W-2 forms and schedules as well as pay stubs for the specified years to the court and the opposite party.
When is the Financial Declaration form due?
The Declaration is filled out when the petitioner files a case against you. After the declaration is served, the respondent must complete it as soon as possible.
What information should be provided in the Financial Declaration form?
The declaring has to fill in the following sections:

Summary of basic information (total monthly net income; household expenses; debt expenses; estimate of the other party’s gross monthly income)

The declaring must certify the form with a signature and date it.
What do I do with the completed form?
The Declaration is filed with the court. One copy is forwarded to the petitioner.
